Making an Appointment.

At Washington Hand Surgery we want to make scheduling appointments as simple as
possible. One of our knowledgeable receptionists will help you contact the physician best trained
to meet your specific needs. The fastest way to make an appointment or to change an existing appointment is to call our main numbers:

New Patient Registration Forms

For your convenience, our patient registration forms are available for printing. Please complete these forms and bring them to your first office visit.

Please print and complete the following five (5) forms:

Patient Registration
Paient Health History
Financial Policy
Notice of Privacy Practices (HIPAA)
Injury Questionnaire

As a new patient, please arrive 15 minutes prior to your scheduled appointment and bring:

• Driver’s license or other photo ID with your current address
• Actual X-ray or MRI films and reports of the problem area. THESE ARE VERY IMPORTANT *
• Any notes from your doctor, including reason for consultation. *
• All testing related to your problem. *
• List of medications.
• All insurance and billing information, including insurance card and the address of the insurance company. If this is work related we will need your claim number, insurance company that handles injuries on the job, their phone number and billing address.
• Please call your insurance company BEFORE your appointment to verify that we are a preferred provider. If your insurance company requires a referral, this must me obtained prior to your appointment.
• Your co-payment will be collected at the time of service. There is a $20.00 service fee if not paid on the day of your visit.
• If you do not have all of the above items with you, we will, unfortunately, have to reschedule your appointment.
